I agree that by signing this form I accept the following Terms & Conditions:

  • Fees are due within two weeks of commencement of the term, and will be paid in accordance with this;
  • Fees are non-refundable and will not be remitted in the event of absence from classes;
  • One full terms notice, in writing, is required for cessation of attendance of classes. If a full terms notice is not provided then fees will be due for the following terms lessons;
  • Students cannot be considered for exams or performances unless fees are fully paid and up to date prior to the date of the exam or performance;
  • Entry into any exam session is at the full discretion of the dance teacher;
  • Whilst it is kept to a minimum, to ensure safe dance practice, dance training involves some forms of physical contact. Signature of this form acknowledges and gives consent for this;
  • The dance school cannot take responsibility for any personal belongings left on the premises, either before during or after lessons or performances or exams;
  • Inappropriate, violent or aggressive behaviour will not be tolerated and will be dealt with accordingly;
  • Parents are not permitted to view classes, except on designated viewing days, and the school reserves the right to remove any parties from the premises that do not have permission to be present.

By submitting this form you are accepting to submit your data to us for the purpose of enroling your child as a student at Forrestall School of Irish Dancing. This data is used to ensure the safety of your child whilst in the care of the School and it's teachers. We will not use this data for any other purposes and the data will be processed in line with the General Data Protection Regulations (GDPR). Photo & Video Consent Form

This consent is valid for the Forrestall School 2026/27 dance year.

At Forrestall School of Irish Dancing, we occasionally take photographs and videos of dancers during lessons, performances, competitions, workshops and other activities or events connected with the school.

We may use these photographs and videos to celebrate and promote Forrestall School and Irish dancing. This may include use on the Forrestall School website, social media pages such as Instagram and Facebook, printed materials, press or media features, fundraising and sponsorship applications, and other promotional material relating specifically to Forrestall School of Irish Dancing.

We take the privacy, safety and wellbeing of our dancers seriously. Photography and video will be carried out in accordance with our safeguarding responsibilities, UK GDPR and the Data Protection Act 2018, and the rules and Child Protection Policy of An Coimisiún Le Rincí Gaelacha (CLRG).

DANCER'S DETAILS
Consent Terms & Conditions

I/we give permission for Forrestall School of Irish Dancing to take photographs and/or video of my child during dance lessons, performances, competitions, workshops and other dance-school-related activities and events during the 2026/27 dance year.

I/we give permission for Forrestall School of Irish Dancing to use photographs and/or video in which my child appears for the purposes of promoting and publicising Forrestall School of Irish Dancing and Irish dancing.

This may include:

The Forrestall School website Forrestall School social media pages, including Instagram and Facebook Printed school publicity and promotional materials Press releases and media features relating to the school Fundraising and sponsorship applications or presentations for Forrestall School Promotional material relating to Forrestall School events and achievements

Images will only be used for purposes connected with Forrestall School of Irish Dancing and will not knowingly be sold or licensed for unrelated commercial purposes.

CLRG requirements

I understand that Forrestall School of Irish Dancing is registered with An Coimisiún Le Rincí Gaelacha (CLRG) and that photographs and videos used on social media must comply with CLRG rules and its Child Protection Policy.

I understand that CLRG requires written permission for the inclusion of individual dancers in social-media posts. I understand that this consent may be made available to CLRG if requested.

I understand that giving consent does not mean that every photograph or video will be published. Forrestall School will use its discretion when selecting images and will ensure that dancers are appropriately attired and portrayed positively and respectfully.

Privacy and safeguarding

Forrestall School will take reasonable steps to ensure that photographs and videos are stored securely and handled in accordance with applicable UK data protection and safeguarding requirements.

We will not knowingly publish unnecessary personal information alongside an image that could compromise the safety or privacy of a child.

Where appropriate, group photographs and general school activities may be used without identifying individual dancers by name.

Withdrawal of consent

Consent may be withdrawn at any time by contacting Forrestall School of Irish Dancing.

Withdrawal of consent will apply to future use of images. We will make reasonable efforts to remove images from our own digital platforms following withdrawal of consent, but we cannot guarantee the removal of material that has already been printed, distributed, published by third parties or shared by others.

Declaration

I confirm that I am the parent/guardian of the dancer named above and that I have read and understood the information provided.

By ticking the box below, I confirm that I give Forrestall School of Irish Dancing permission to photograph and/or video my child and to use those images for the purposes described above throughout the 2026/27 dance year.
